Application for Banded Hours

If you wish to avail of a new banded hour contract and you have more than 12 months service and have not applied for a banded hour contract in the last 12 months then please complete the form below:

I am writing to request that I be placed in a new band of weekly working hours as per Section 18A of the Organisation of Working Time Act.  

I look forward to the prompt processing of my request as per the Act.
